Case Management Services

The JFS Case Management Department encompasses a broad-reaching, comprehensive approach to services with the goal of helping individuals and families improve the quality and direction of their lives.

What is Case Management?

Case management can be many different things to many different people.  It can be a simple as providing information or making a phone call.  It can be as involved as a lengthy relationship focused on improvement in many life areas.  In its most basic form, case management includes conducting a needs assessment, developing goals, providing information, making referrals, connecting to vital community resources, coordinating important services, advocating for services and benefits, and maintaining follow-up contact with both clients and the community services for those clients. 

How Can Case Management Help?

Case management is most helpful for individuals and families who find it difficult, for whatever reason, to maintain or improve their quality of life.  By conducting a thorough needs assessment, a case manager is able to help outline a plan with specific steps for meeting the identified needs.  Sometimes those needs may be able to be addressed by JFS while at other times those needs might be better  addressed by some other agency, service provider, or community resource.  Either way, the case manager is the conduit to securing those services and starting the process of getting those needs met.

The JFS case manager can help coordinate services related to education, community resources, guidance, and assistance in the following areas, just to name a few:

- Government  & community benefits
- Financial assistance
